SSIB Annual Meeting Registration Form


Everyone who plans to attend the SSIB annual meeting is requested to register online using this form.  Online registration sends your contact information directly to the SSIB Central Office.  Registration insures that you will receive any updated information on the meeting including any updates to the program and reminders of important deadlines.  Please contact the Central Office if you have any questions.

NOTE:  Please complete and SUBMIT  the registration form before continuing to make your payment.  If you do not, you will not be registered.  Registration and making payments are two separate functions therefore you may register now and make your payment later if you wish.
